Does a Benign Thyroid Nodule Always Require Surgery?

When a thyroid nodule is found, many people worry and immediately think of surgery. In reality, the majority of thyroid nodules are benign and do not necessarily require an operation. Treatment depends on the nature of the lesion, its size, symptoms and the patient’s preferences.

When does a thyroid nodule only need monitoring?

If a nodule is benign (confirmed by fine needle aspiration when needed), small and causing no symptoms, the doctor will usually recommend periodic ultrasound monitoring every 6–12 months. Many thyroid nodules persist for years without affecting your health.

Minimally invasive treatment options

When a benign thyroid nodule causes compression, discomfort or cosmetic concerns, the following options may be considered:

• Radiofrequency ablation (RFA): uses heat to shrink the nodule while preserving thyroid tissue, leaving no scar.
• Absolute ethanol injection: highly effective for benign thyroid cysts, helping them gradually collapse.

These two methods are usually performed on an outpatient basis, without general anaesthesia, and with a quick recovery.

When is surgery needed?

Surgery is considered when a lesion is suspected to be malignant, when a nodule is too large and causes severe compression, or when minimally invasive methods are not suitable. Your doctor will give specific advice based on the characteristics of the lesion and your overall health.

Why is evaluation needed before treatment?

Correctly identifying the nature of the lesion is the most important step. Ultrasound findings combined with fine needle aspiration help distinguish benign from malignant lesions, avoid unnecessary treatment and select the right approach.
